Free Eyeglasses with Medicaid: woman wearing glasses smiling in an eyewear store

Children on Medicaid can get medically necessary eyeglasses through EPSDT. For ages 19–64, eyeglasses are an optional benefit, so coverage depends on your state and plan.

Children

Enrolled children and young people under 21 have vision services under EPSDT, including glasses when needed.

People

The federal Medicaid benefits list lists eyeglasses as optional. Some states cover glasses at set intervals, some only with certain conditions or eligibility groups, and some don't cover them for most people aged 19–64.

How to use the benefit

  1. Call your plan and ask what is covered and how often.
  2. Ask for an eye doctor who accepts your plan.
  3. At the store, ask which frames are fully covered before you choose. Upgrades can cost extra.

If it isn't covered

Try a local Lions Club or a school of optometry, or check with OneSight or New Eyes. Related: benefits for children and free rides to appointments. Check your eligibility.

What to ask at your eye appointment

Before your visit, ask if the exam and the glasses are both covered, as they can be separate benefits. Ask how to replace lost or broken glasses, especially for children. Medical eye care, such as treatment for diabetes-related eye problems, may be covered even when routine glasses are not.

How to check what your plan offers

  1. Call the member number on your Medicaid card.
  2. Ask for the member handbook, and look for "extra" or "value-added" benefits.
  3. Ask about limits, approvals and which stores or providers you must use.
  4. Get the answer in writing, if you can.

State and plan benefits can change every year, so ask again at renewal. If you're moving, check your new state's Medicaid calculator page, because coverage doesn't transfer automatically.
